ORNAHD recognizes that privacy of information is important. This privacy notice is to explain to you what ORNAHD does with information and the manner in which it is used. This notice is to voluntarily comply with federal Personal Information Protection and Electronic Documents Act (PIPEDA).
Collection and Use of Personal information
ORNAHD limits its requests for information to ONLY what is required to ensure ongoing communication and completion of work for the Operating Room Nurses Association of Hamilton & District (ORNAHD). Most of the personal information collected by ORNAHD is for use within the association. The information is basic including but not limited to, name, address, telephone number, fax number, email address and work phone number, address, and job title.
Internet
The ORNAHD web site lists the Executive. Your email address or other information shall not be posted without your knowledge. The number of visits to our website is monitored. Inquiries from the website are dispersed to appropriate members or Committee Chairs via the web monitor. The member may respond directly to the inquirer or back to the web monitor as they wish.
Corporate Background
ORNAHD is a volunteer organization within perioperative registered nursing practice, therefore is limited in corporate activity and clients. ORNAHD clients will be asked for information pertaining to the work involved and shall not share it with others. Clients shall be informed of information sharing in advance.
Individual members of the Executive have the right to refuse posting of personal information in ORNAHD venues, including but not limited to, Canadian Operating Room Nursing Journal (CORNJ), www.ORNAHD.org, or advertising such as event posters.
Disclosure of Personal Information
ORNAHD does not share information. Member lists are not shared on a for profit basis, nor are
event registrant lists. ORNAHD will retain information for as long as it is required for the
functioning of the association and shall not share without your permission. For example, ORNAHD will
not give your information to another individual or group to mail information to.
By becoming an ORNAHD Executive member we, ORNAHD, assume this information may be
shared and listed in such things as the Canadian Operating Room Nursing Journal (CORNJ) where
you may be listed as a Regional Board member.
Accuracy of Information
ORNAHD relies on our members to notify the association of changes to personal information.
Protection of Personal Information
ORNAHD shares personal information with only those needing the information. Personal information is reviewed bi-annually at Executive Meetings. Members shall note such items as unlisted phone numbers or designate a "use First" policy with restricted information.
Access to Personal Information
ORNAHD is committed to ensuring we gather only information we need about you.
Filing of a Complaint
Should you be dissatisfied with ORNAHD's privacy policy or practices regarding privacy we shall investigate your complaint and take corrective actions if needed or it is felt to be necessary. All complaints shall be shared and reviewed with the Executive. A report shall be given to you. If you remain dissatisfied you may send a report to the federal Privacy Commissioner.
